Much of your medical records can be downloaded and printed from our MyChart patient portal. However, you may also request this information through one of the online forms linked below:

Frequently asked questions
How do I request a copy of my medical records?
You can request your medical records through MyChart, either in the app or on a computer.
You can also request through our online portal.

How long does it take to receive my records?
Federal law gives us up to 30 days to send you your records once we receive your request. We will do our best to get them to you sooner.
If we need more time, we will send you a written notice. In that case, we may take up to 30 additional days, for a total of up to 60 days.
Is there a fee for requesting records?
UAB Medicine does not charge patients for copies of their own medical records.
However, federal and state law do allow us to charge a small fee when records are sent to a third party, such as an attorney or another organization, with your permission.
Can I access my records through MyChart instead of requesting them separately?
Some of your records are available to view directly in MyChart. Right now, you can see a portion of records created in our new system.
If you need records that are not yet visible in MyChart, you can submit a request and we will provide them.
Who can request my records on my behalf?
Federal law allows certain people to request your records on your behalf, as long as they have the proper documents. These include:
- A Health Care Power of Attorney (HCPOA)
- A Durable Power of Attorney that names a health care agent
- Court-ordered guardianship documents
- A parent or legal guardian of a child under the age of 19
These documents help us protect your privacy while making sure the right people can access your information.
How do I submit a correction if I believe something in my records is wrong?
If you think something in your medical record is missing or incorrect, you can ask us to fix it. Just contact the Compliance Department at 205-975-0585 to start the process. You’ll need to put your request in writing, and your provider will review it to decide whether to make the change.
If we’re not able to make the change you asked for, we’ll send you a letter explaining why. You’ll still have the right to add your own written statement to your record, explaining that you disagree with what’s documented.
How long are my records kept?
Medical records are kept according to state and federal law. Medical records are kept for at least 10 years. Some records, such as records for minors, may be kept longer based on legal or clinical requirements.

Can I request records from before the Epic transition?
Yes. You can request records created before we moved to the Epic system. You can submit your request:
- Through MyChart (use the “”Additional Comments”” section to note that your request is for pre-Epic records)
- In person at our office
- Through the patient request portal
- By mailing or faxing a completed patient authorization form
Are all my medical records available in the portal?
Some of your records are available to view in MyChart right now. At this time, you can see a portion of records created in our new system.

Can I download medical records from the portal?
Yes. You can download records that are available to you in MyChart as a PDF.
Can I request billing records through MyChart?
You can view itemized billing statements in MyChart for services you received on or after July 25, 2026.

Can I request Radiology images through My Chart?
Radiology images are not available to view directly in MyChart at this time. However, you can request copies of your radiology images through MyChart by using the “Additional Comments” section of the medical records request form.

The move to Epic also gives you a new option: you can now view many of your records in one place through the MyChart app.
How do I specify records from a particular location?
You can request records from any UAB St. Vincent’s location through MyChart by using the “Additional Comments” section of the records request form. To help us find your records quickly, please include:

- Type of treatment
- Provider name or location

How will my personal health information be protected?
We take your privacy seriously. Your health information is personal, and we are committed to protecting it.

What happens if my request is incomplete or missing information?

How are urgent or time-sensitive requests prioritized?
If your records are needed for ongoing medical care by a provider outside of UAB Medicine, you can ask that provider to request them on your behalf. These requests are given priority and are typically fulfilled within 24 to 48 hours after we receive them.
All other requests are handled in the order received. Federal law allows up to 30 days to complete them.
Are requests handled differently for hospitals versus clinics?
No. All UAB Medicine locations and departments follow the same process and timeline for medical record requests. By law, we have up to 30 days to complete your request. If we need more time, we’re allowed up to 60 days total, but we’ll notify you in writing if that happens.
