As a patient of UAB St. Vincent’s, it is your right to receive a copy of your medical record and/or request that your medical record be released to a third party.

Much of your medical record can be downloaded and printed from our Patient Portal.

Is there a cost for patients to receive their own medical record?

No. Also, there is no cost for having your medical record sent to another health care provider.

How long does it take to process my medical record request?

According to federal law, we have 30 days to respond to your request. However, we do our best to get the record out sooner. If you requested that your record be mailed to you, please allow enough time for postal service delivery.

How long are records maintained at UAB St. Vincent’s?

Our policy is to maintain them for 10 years. After 10 years, records may not be available.

Can someone other than the patient request or receive copies of their medical record?

Yes, but only if the person signing on behalf of the patient has provided supporting documentation (i.e. advance directive/health care power of attorney) stating that the person making the request is authorized to do so.

How do I request the medical record for a patient under age 19?

If the patient is a minor or is unable to provide consent, the signature of a parent, guardian, or other legal representative is required, along with documentation of legal authority (if requested by UAB St. Vincent’s).

How do I obtain a copy of a deceased person’s medical record?

The request must be accompanied by authorization from the person’s executor, executrix, administrator of the estate, or personal representative, along with documentation indicating legal authority. A death certificate might also be requested.

How do I correct my medical record if I believe the information is incomplete or inaccurate?

You have the right to submit a formal written amendment request, then your provider may either approve or deny the request. For more information on requesting an amendment to your record, please call our Compliance Department at 205-975-0585.
