Frequently asked questions

How do I request a copy of my medical records?

You can request your medical records through MyChart, either in the app or on a computer.

You can also request through our online portal.

If you prefer, you are welcome to visit our office in person or mail your request to us. See contact information for locations and mailing information. Click here for a downloadable form to request your records.

How long does it take to receive my records?

Federal law gives us up to 30 days to send you your records once we receive your request. We will do our best to get them to you sooner.

If we need more time, we will send you a written notice. In that case, we may take up to 30 additional days, for a total of up to 60 days.

Is there a fee for requesting records?

UAB Medicine does not charge patients for copies of their own medical records.

However, federal and state law do allow us to charge a small fee when records are sent to a third party, such as an attorney or another organization, with your permission.

Can I access my records through MyChart instead of requesting them separately?

Some of your records are available to view directly in MyChart. Right now, you can see a portion of records created in our new system.

If you need records that are not yet visible in MyChart, you can submit a request and we will provide them.

Who can request my records on my behalf?

Federal law allows certain people to request your records on your behalf, as long as they have the proper documents. These include:

  • A Health Care Power of Attorney (HCPOA)
  • A Durable Power of Attorney that names a health care agent
  • Court-ordered guardianship documents
  • A parent or legal guardian of a child under the age of 19

These documents help us protect your privacy while making sure the right people can access your information.

How do I submit a correction if I believe something in my records is wrong?

If you think something in your medical record is missing or incorrect, you can ask us to fix it. Just contact the Compliance Department at 205-975-0585 to start the process. You’ll need to put your request in writing, and your provider will review it to decide whether to make the change.

If we’re not able to make the change you asked for, we’ll send you a letter explaining why. You’ll still have the right to add your own written statement to your record, explaining that you disagree with what’s documented.

How long are my records kept?

Medical records are kept according to state and federal law. Medical records are kept for at least 10 years. Some records, such as records for minors, may be kept longer based on legal or clinical requirements.

If you have questions about whether a specific record is still available, please contact the Medical Records department at 205-930-7724.

Are all my medical records available in the portal?

If your records are needed for ongoing medical care by a provider outside of UAB Medicine, you can ask that provider to request them on your behalf. These requests are given priority and are typically fulfilled within 24 to 48 hours after we receive them.

All other requests are handled in the order received. Federal law allows up to 30 days to complete them.

Are requests handled differently for hospitals versus clinics?

No. All UAB Medicine locations and departments follow the same process and timeline for medical record requests. By law, we have up to 30 days to complete your request. If we need more time, we’re allowed up to 60 days total, but we’ll notify you in writing if that happens.

What if I submitted a medical records request before the transition to MyChart on July 25, 2026 — will it still be processed?

Data integrity

How does the organization prevent duplicate medical records?

UAB Medicine uses several steps during registration to make sure each patient has only one medical record. As we bring all of our facilities together under one system, it is more important than ever that we confirm your information at each visit — including your name, date of birth, address, and phone number. These steps help make sure your health information is accurate and available to the right care team.

What should I do if my personal information changes?

If any of your personal information changes, please let us know as soon as possible. This includes changes to your:

  • Name
  • Address
  • Phone number
  • Email address
  • Insurance information
  • Emergency contacts

Keeping your information current helps make sure your medical record is accurate, your bills go to the right place, and our team can reach you when needed. You can update your information during registration, at check-in, through MyChart, or by calling your provider’s office.

How will my information be verified during registration or check-in?

When you check in or register for an appointment, a staff member will ask you to confirm key information to make sure your record is accurate and up to date. You may also be asked to show a photo ID and insurance card.

This step protects your privacy, supports safe care, and helps make sure all of your health information is linked to the correct record.

How does the organization protect my personal and health information?

UAB Medicine takes multiple steps to keep your personal and health information safe, including:

  • Limiting access to staff who need it to do their job
  • Using secure systems with passwords and encryption
  • Monitoring system access through regular security reviews
  • Training staff on privacy and confidentiality requirements
  • Following all federal and state privacy laws, including HIPAA
  • Using secure methods when sharing information with other providers or health plans

These measures help make sure your information stays private and is only used to support your care.

Who should I contact if I notice incorrect information in my chart?

If you notice something in your chart that does not look right, please contact your provider’s office or our Compliance Department at 205-975-0585 for help.

Will I need to provide identification more often?

Yes, we may ask you to verify your identity at each visit, even if you have been a patient with us before. This is an important step to protect your health information and make sure we are working from the correct medical record.

Can errors in my medical record affect billing or treatment?

Yes. Errors in your medical record can affect both your care and your bill. Incorrect or outdated information may:

  • Cause delays in your care
  • Lead to decisions being made based on incomplete information
  • Result in billing or insurance claim problems
  • Cause statements to be sent to the wrong address
  • Create duplicate records that make it harder for your care team to see your full health history

If you notice an error in your record, please let us know right away.
